First and Last name
Department Name, City & State (if a large agency, include precinct/unit/etc)
Department Phone Number
A date and time that you can be contacted

Once you get verified you'll be able to answer questions in the Ask-A-Cop section as well as access the Verified side where the conversation gets a bit more specific.
