Do you need to take this test every year? Or is it a one time thing?

With all the local departments I know, it's a one time thing upon application. If it becomes a problem later, they may ask you to retest, but that would be rare. If you work for a civil service type department, you wouldn't lose your job even then, but be put somewhere where you can function or retire you.
