    Question in regards to application

    I received my complete work history from the Social Security Administration (S.S.A.) When I fill out applications, should I use the addresses listed on the form? Some of the addresses listed are the main office which is not the actual physical location in which I worked. There is also one company that I worked for that falls under another name.

    When filling out applications, I have been using the address of the actual physical location. Also, I am in the academy. I notice on some department websites they have applications for certified and noncertified officers. Which one would I fill out?

    Use the address that suits you, as long as they are able to contact the company, you should be okay

    That is what I thought. But one of our instructors was talking about a position and said to fill out the certified one, because when you're done that is what you will be. Thanks for the advice.
