    Personal History Statement

    Question reads "list all traffic citations you have ever received by issuing agency and disposition" My question is, who should I put for issuing agency if I was given a warning by the highway patrol? Or did I just answer my own question? haha
    Also, what do they mean by disposition? Any help would be greatly appreciated.

    Welcome to the forum. We ask that new members introduce themselves in the appropriate area if they intend to stick around.

    As to your question:

    A warning is not a traffic citation, so I would not list that at all.

    If I *was* going to list it, the Highway Patrol would be the agency.

    Disposition would be "plead guilty" "found guilty at trial" "acquitted" "charges dropped" "amended to loud muffler" etc.
