    BI question

I am in the middle of filling out my BI packet for Camden, NJ. I used to work for a large retailer in 2001. After working their for about two months, I was offered a job with another company that I had interviewed with months earlier. My boss's secretary overheard me speaking on my cell phone about the new position with the recruiter. I had a vacation planned and was planning on telling my boss I was to resigning after my vacation. I came in after my vacation and my boss called me into the office with her boss. I told her that I was planning on leaving and found a new a new position. She then told me that I was not truthful on my dates of employment from my previous employer and that I could not get rehired. I did not even press the issue, since I got the job I wanted. I got my file from the company and it says termed for falsification on application. However this was not true. I resigned. I requested my application and they said they do not have a copy of it. I am not sure what to put down as a reason for leaving. They never said you were fired etc. I would say mutually agreeded seperation anything. You can see that I started shortly after with my current company. Would it be best to give this paperwork to my BI or just let him call and find out. They are not requesting the info. I have been working at my present employer which in fortune 100 company where I have been a high level manager for the past five years and have wonderful reviews to show how well I perform. Do you think this will DQ me? Otherwise I have clean record except for a few speeding tickets. Thanks for your help.
