I would like to ask for your help for an assignment. what does sj, R, V, and I mean in the incident report?
and what is the purpose of a "basic understanding of truth and lies" in a supplemental report?

You will need to provide some context. Those abbreviations are not anything used in reports in my area.

We use those abbreviations etal but they may mean something different in your particular case. Please scan and post the incident and supplemental reports.

(Speed) S= 5.5 divided by the square root of distance times velocity.

(Radius) R= Cord squared divided by 8 times Middle Ordinate plus Middle Oridinate divided by 2.

sj = shock jock ?

Normally that's used when you're dealing with young children in order to confirm that they know the difference between a truth and a lie. If a child can't differentiate between the two, there can obviously be issues with their testimony.
